How to get started?

To enter the development program:

1. Schedule a tour with Ms. Trisha to see our classrooms and meet our teachers!
2. If you child has a diagnosis of Autism spectrum disorder, Muscular dystrophy, Cerebral palsy, Down syndrome, Phelan McDermid syndrome, Prader-Willi syndrome, Spina bifida, Williams syndrome, and/or cognitive disability, go to sufs.org and apply for the Gardiner scholarship. This will pay for your tuition to our program as well as other educational services.  (The scholarship is available to children starting at age 3 who are not enrolled in the public-school system)
3. Gather any paperwork that we may need to best serve your child. This may include, referrals for therapy, previous IEP’s or therapy evaluations, and/or diagnostic testing from the doctor.
4. Fill out our application online and other applicable forms for any services that you are interested in.

To receive OT/PT/Speech Therapy:

1. Fill out our program enrollment form and permission to bill insurance form under our forms tab.
2. Bring in copy of your insurance card.
3. Bring a copy of a doctor’s referral for the applicable therapy.
4. Fill out the medical health questionnaire and sensory profile under our forms tab.
5. For Physical Therapy, fill out the PT intake form under our forms tab.

To receive ABA Services:

1. Obtain a referral from the doctor the services
2. You must have a diagnostic report from a school district with testing data given in the evaluation, or formal testing from a neuropsychologist. This is very important!  Insurance will not allow for an evaluation without this documentation.
3. Fill out the Functional Behavior Assessment under our forms tab.

Call our office at 727-378-4182 to schedule a parent interview and child observation.  This can only be scheduled if steps 1-3 have been done.  We are often on a waiting list for ABA so call asap once the appropriate documentation is obtained.
